This is an exciting opportunity for an experienced Adults Social Worker to join a dynamic Adult Social Care service in Lewisham. You will play a key role in supporting vulnerable adults, promoting independence, and ensuring safeguarding responsibilities are met in line with statutory duties.

What you will do

• Complete Care Act 2014 assessments and reviews using a person‑centred approach.
• Carry out Mental Capacity Act assessments and make Best Interests decisions when required.
• Manage a varied caseload of adults with complex needs, including older people, those with physical disabilities, and individuals with mental health needs.
• Develop, implement, and review personalised care and support plans.
• Lead safeguarding enquiries in line with Section 42 requirements.
• Work collaboratively with health professionals, care providers, and partner agencies.
• Support safe and timely hospital discharge processes and community‑based interventions.
• Maintain accurate, detailed, and timely case records in line with statutory guidance.
• Promote independence, wellbeing, and positive outcomes while managing risk effectively.

Requirements

Essential:

• Qualified Social Worker (DipSW, CQSW, BA/MA Social Work).
• Registered with Social Work England.
• Minimum of 2 years post‑qualification experience in Adult Social Care.
• Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.
• Experience managing complex caseloads and producing high‑quality assessments and reports.

Desirable:

• Experience working within a Local Authority Adult Social Care team.
• Familiarity with systems such as Mosaic or Liquidlogic.
• Experience in hospital discharge or integrated teams.
• AMHP or BIA qualification.
