An exciting opportunity has arisen for an experienced Adults Social Worker to join the Hospital Team at Southampton City Council in Southampton. This ongoing locum role offers the flexibility of part-time or full-time work while focusing on supporting adults within hospital settings to ensure timely and person-centred discharges.

This role provides the opportunity to make a tangible difference in the lives of individuals at a pivotal stage in their recovery. You will work closely with health professionals, families, and community services to assess needs, arrange care packages, and promote independence while reducing delayed hospital discharges.

What You Will Do

  • Conduct strength-based assessments for adults within hospital settings to understand their needs and support requirements.

  • Complete Care Act assessments and develop tailored care and support plans.

  • Facilitate safe and timely hospital discharges in partnership with multidisciplinary teams.

  • Undertake Mental Capacity Act assessments and make Best Interest decisions where appropriate.

  • Work closely with health professionals, discharge coordinators, and community services to coordinate care.

  • Arrange care packages, placements, and community support services to meet ongoing needs.

  • Manage and respond to safeguarding concerns for vulnerable adults.

  • Maintain accurate case records and reports in line with local authority standards.


Requirements

  • Qualified Social Worker registered with Social Work England.

  • Experience working within adult social care or hospital discharge teams.

  • Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.

  • Ability to work effectively in a multidisciplinary hospital environment.

  • Excellent assessment, communication, and case management skills.


Desirable Experience

  • Previous experience in hospital discharge or integrated discharge team settings.

  • Knowledge of Continuing Healthcare (CHC) processes.

  • Experience coordinating complex care packages.
