What you will do:

• Apply your community rehabilitation expertise by working collaboratively with patients who have cognitive and physical difficulties.
• Devise and implement effective care plans and home exercise plans tailored to individual needs.
• Conduct careful assessments of patients in their homes and issue necessary equipment.
• Provide elderly care with a focus on improving quality of life and independence.
• Exhibit excellent assessment and rehabilitation skills, ensuring that your patients receive the highest level of care.
• Manage complex manual handling situations with confidence and care.

To thrive in this role, it is essential to have at least two years of experience and be HCPC registered. While being a car driver is preferable, it's not mandatory.
