What you will do:
- Conduct initial assessments of prisoners to identify their substance misuse and mental health needs.
- Develop personalized recovery plans in collaboration with prisoners, focusing on achievable and measurable recovery goals.
- Deliver 1:1 and group recovery-focused interventions, using evidence-based therapeutic approaches.
- Monitor progress and adjust recovery plans as necessary.
- Provide crisis support and ensure appropriate referrals are made for individuals requiring additional services.
- Plan and facilitate group sessions aimed at improving recovery knowledge, building life skills, and supporting long-term sobriety.
- Deliver substance misuse treatment programs, including educational workshops, relapse prevention strategies, and harm reduction techniques.
- Help prisoners address underlying issues contributing to their addiction, such as trauma, abuse, or family issues.
- Work in partnership with prison officers, healthcare providers, and external agencies to coordinate comprehensive care for prisoners.
- Attend multidisciplinary team meetings and contribute to case reviews and care planning processes.
- Assist in preparing prisoners for release, ensuring they have access to post-release support, including housing, employment, and ongoing addiction services.
- Maintain accurate and up-to-date records of prisoner progress, treatment plans, and interventions.
- Prepare reports on client outcomes, attending case reviews and contributing to performance audits.
- Participate in ongoing training and professional development to keep up-to-date with the latest recovery practices and legislative changes.
