What you will do:

- Conduct home visits to assess clients' needs and develop tailored care plans that promote independence and wellbeing.
- Perform assessments to identify necessary equipment and adaptations to improve clients' daily living activities.
- Collect small equipment such as bath boards, toilet frames, and commodes from a local store; deliver and install these items while demonstrating their safe use to clients.
- Collaborate with a multidisciplinary team to ensure holistic and effective care for clients.
- Maintain detailed records and documentation to track clients' progress and support service outcomes.

Requirements:

- HCPC registration is essential for this role.
- A valid driver's license is necessary as you will need to travel to various client locations and pick up equipment.
