What you will do:
- Review prescriptions to ensure accuracy and legality, identifying and resolving any actual or potential problems.
- Work collaboratively with the healthcare team to optimize the pharmaceutical management of patients in medical and surgery wards.
- Provide expert advice on medication management to both patients and healthcare professionals.
- Conduct medication reconciliations and discharge planning to ensure seamless patient care.
We are looking for a GPhC-registered pharmacist with at least two years of relevant experience, ideally in a hospital setting. Having your own transport is preferable due to the multi-site nature of the role.
