What you will do:

Complete comprehensive and risk assessments for adults and young people presenting with substance misuse issues
Develop and review individual recovery plans, measuring progress against agreed outcomes
Deliver a full range of one-to-one and group interventions, including harm reduction advice and structured recovery support
Liaise with key partners, including criminal justice agencies, schools, health professionals, and social care, to ensure a holistic, multi-agency approach
Attend multi-professional meetings and contribute to joint care planning
Maintain accurate and timely records using Illy Care Path
Support clients to make positive changes, promoting recovery, resilience, and community reintegration
