Overview
We are seeking a qualified Social Worker to join the Adults Hospital Discharge Team in Rochdale. This ongoing locum role offers the opportunity to work across multiple hospital sites, including out-of-borough locations, facilitating safe and timely discharges for adults and older people.

Key Responsibilities

• Conduct comprehensive assessments using a strengths-based approach to evaluate adults’ and older people’s social care needs.

• Facilitate safe and timely hospital discharges, ensuring appropriate support arrangements for individuals and their carers.

• Develop innovative care plans that meet assessed needs, incorporating both universal services and natural support networks.

• Promote the use of direct payments to empower individuals to manage and purchase their own care.

• Ensure compliance with professional standards and statutory requirements under the Care Act 2014 and the Mental Capacity Act 2005.

• Build and maintain effective working relationships with council services, statutory agencies, and third-sector providers.

• Conduct mental capacity assessments, safeguarding the best interests of vulnerable individuals.

• Commit to continuous professional development and adherence to the Professional Capabilities Framework.
