Locum Opportunity: Outreach Worker in Northampton, UK, £23 Hourly - 3 months with a view to extend

What you will do:

• Engage with individuals experiencing street-based living, including rough sleepers and beggars, connecting them with the right services and housing options
• Empower service users to achieve sustainable change through personalised and coordinated support
• Focus on building individuals’ strengths and social networks, both locally and in their home communities
• Deliver flexible, assertive, and creative interventions to engage hard-to-reach individuals and help reduce homelessness
• street outreach shifts at varying times and locations to identify and engage rough sleepers
• Provide assertive outreach to support entrenched rough sleepers, including those initially resistant to engagement
• Collaborate with housing, healthcare, and social care services to develop holistic, person-centred plans
• Assist individuals being discharged from hospital, ensuring rapid access to safe and appropriate accommodation
• Maintain accurate records, report progress, and contribute to multi-agency meetings and local homelessness initiatives
