Key Responsibilities

• Undertake assessments of need and risk, applying the principles of the Care Act 2014 and other relevant legislation.

• Develop and implement person-centred support plans that promote wellbeing and independence.

• Work collaboratively with health professionals, voluntary agencies, and other partners to deliver integrated and effective services.

• Manage and prioritise an allocated caseload, ensuring all statutory responsibilities and deadlines are met.

• Maintain accurate and up-to-date records in line with professional and organisational standards.

• Participate in supervision, training, and team meetings, contributing to reflective practice and continuous service improvement.

• Support the professional development of others, including students, where appropriate.

About You

You will be a confident and compassionate Social Worker who demonstrates excellent assessment, communication, and problem-solving skills. You will have experience managing complex cases and working effectively in a multi-agency environment.

Essential Criteria:

• Degree or Diploma in Social Work (or equivalent qualification).

• Current registration with Social Work England.

• Substantial post-qualification experience within Adult Social Care.

• Good working knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures.

• Ability to work independently while maintaining a collaborative approach within a supportive team.

• Access to a car and ability to travel across the Cheshire East area.

Desirable Criteria:

• Experience in frontline statutory social work.

• Familiarity with local authority processes and electronic case management systems.
