What you will do:

- Conduct comprehensive Care Act assessments, support planning, and risk assessments.
- Work under the Care Act 2014, performing mental capacity assessments and drafting social circumstance reports.
- Collaborate effectively within a well-established team, maintaining close working relationships with the community mental health team and other health professionals.
- Operate independently when necessary, showcasing your proficiency in mental health social work and your knowledge of the Mental Health Act.
- Carry out safeguarding enquiries, working in a strengths-based approach to support individuals.

This role demands skill in facilitating Care Act assessments and utilises your knowledge of mental health to enhance support planning and risk management.
