Flintshire County Council is seeking a Social Worker - Hospital Discharge Team within the Adults group, to work on a temporary basis in Mold. You will be working full time, earning £33.55 per hour as a Limited or Umbrella worker, or £25.58 per hour for PAYE, for a duration of 6 weeks. Your role will focus on providing crucial support during the hospital discharge process, ensuring that patients receive the care and guidance they need.

What you will do:

  • Conduct community care and safeguarding assessments, identifying risks and needs in accordance with statutory requirements.
  • Develop and implement care plans that cater to the individual needs of clients, addressing unmet needs and achieving outcomes.
  • Construct and recommend value-for-money care packages, commissioning services to enhance the lives of service users and carers.
  • Collaborate with individuals, families, carers, and communities to support informed decision-making and self-expression.
  • Manage a caseload of clients, regularly monitoring and evaluating their needs, ensuring resources contribute effectively to the care plan.
  • Cultivate positive relationships with clients, their families, and carers, fostering independence and early intervention where necessary.
  • Liaise with colleagues and external agencies to support comprehensive assessment and care planning, advocating for clients as needed.
  • Maintain accurate case-notes and records, providing reports and court evidence as required.
  • Offer guidance and supervision to social work students, trainees, support staff, or volunteers.
  • Participate in post-qualifying training to ensure continuous professional development and adherence to the standards set by the Care Council for Wales.
