What You Will Do

• Manage a caseload of service users with complex and enduring mental health conditions, supporting recovery and independence.

• Conduct comprehensive assessments and collaboratively develop care plans with clear recovery objectives.

• Deliver evidence-based interventions, including psychosocial support, relapse prevention, and crisis planning.

• Monitor medication as required and work across community settings to champion independent living.

• Act as the named Care Coordinator, reviewing and updating care plans with a focus on positive outcomes.

• Collaborate with multi-disciplinary professionals to ensure cohesive and effective care delivery.

• Involve families and carers in care planning to ensure inclusive support.

• Safeguard adults and children at risk, actively participating in safeguarding processes.

• Contribute to team meetings, offering guidance to junior colleagues.

• Maintain accurate records and engage in continuous professional development.
