What You Will Do

Manage a caseload of service users with complex and enduring mental health conditions, supporting recovery and independence.

Conduct comprehensive assessments and collaboratively develop care plans with clear recovery objectives.

Deliver evidence-based interventions, including psychosocial support, relapse prevention, and crisis planning.

Monitor medication as required and work across community settings to champion independent living.

Act as the named Care Coordinator, reviewing and updating care plans with a focus on positive outcomes.

Collaborate with multi-disciplinary professionals to ensure cohesive and effective care delivery.

Involve families and carers in care planning to ensure inclusive support.

Safeguard adults and children at risk, actively participating in safeguarding processes.

Contribute to team meetings, offering guidance to junior colleagues.

Maintain accurate records and engage in continuous professional development.
