What you will do:
- Support individuals who have been discharged from hospital in regaining their independence, enabling them to remain comfortably in their own homes.
- Visit patients at their residences to perform in-depth needs assessments, implement treatments, and offer valuable advice for a smooth transition back into the community.
- Collaborate closely with a multidisciplinary team, which includes Reablement Support Workers and other healthcare professionals, to design and implement personalised recovery plans for each patient.

Requirements of the Occupational Therapist:
To succeed in this role, you must hold a degree or equivalent qualification in Occupational Therapy and be registered with the HCPC. Recent UK and NHS experience as an Occupational Therapist is essential to meet the demands and expectations of this position.
