What you will do:
- Provide specialist dietetic advice, education, and support to patients and their carers, helping them on their health journeys.
- Offer expert advice, guidance, and support to other healthcare professionals within the team.
- Deliver engaging paediatric outpatient clinics in community-based locations, ensuring accessible and high-quality care.
- Plan, deliver, monitor, and evaluate group-based education sessions in the community for specific patient groups.
- Manage a caseload of paediatric patients who are fed via home enteral feeding, tailoring care to fit individual needs.
- Conduct dietetic clinics at special schools for home enteral feeding patients, fostering a supportive learning environment.
- Triage and prioritise patient referrals to ensure timely dietetic input, in accordance with department standards.
- Lead virtual group-based education sessions to reach a wider audience within the community.
- Provide dietetic input to the neonatal unit at Lister Hospital, facilitating discharge planning for patients on home enteral feeding.
Requirements:
- Must be registered with the HCPC and have at least two years of experience.
- Car driver preferable.
