What You Will Do:

• Coordinate and manage a caseload of individuals with varied complexities, providing tailored health and social care solutions.

• Take a leading role in supporting users and their families, ensuring access to the appropriate expertise and resources.

• Aim to provide efficient, cost-effective services by utilising resources from Social Services, Health, the Council, the community, and non-statutory organisations.
