Contract: Ongoing, block bookings of 3 months (Monday to Friday, 09:00–17:00, with possibility of extension)

What You Will Do

• Assess, plan, implement, and evaluate personalised care programmes in the community.

• Provide skilled nursing interventions such as wound care, catheterisation, injections, and medication management.

• Monitor patients’ conditions, escalating concerns to ensure safety and wellbeing.

• Promote independence and deliver health education to empower patients in managing their own care.

• Collaborate with GPs, therapists, and social care providers to deliver holistic support.

• Support healthcare assistants and students, contributing to team development.

• Maintain accurate patient records and uphold evidence-based best practice.
