What you will do:

  • Assess the physical and mental well-being needs of service users, collaborating with them, their carers, and the multidisciplinary team to plan, implement, evaluate, and monitor personalised care plans.
  • Deliver care that addresses the mental, physical, spiritual, psychological, and social needs of service users, being attentive to individual differences such as age, culture, race, gender, ethnicity, social class, sexuality, and disability.
  • Administer medication with a keen understanding of its contraindications and side effects, providing service users with information in formats they can understand.
  • Ensure up-to-date risk assessments and care plans are available and accurately recorded for all service users attending the clinic.
  • Encourage service users to strive for self-determination and make choices within their legal and ethical boundaries.
  • Always operate within policy, professional, and legal frameworks, including knowledge of the NMC Code of Professional Conduct, risk assessment, child protection protocols, Mental Health Act, and clinical policies.
  • Maintain precise and timely entry of information in the E-PJS, and ensure high-quality written communication in line with Trust protocols and professional standards.
  • Foster open communication and collaborative relationships with colleagues, being an active participant in a supportive and efficient multidisciplinary team environment.
