What you will do:

- Undertake comprehensive assessments under the Care Act 2014 to ensure timely, person-centred support.
- Initiate and progress s.42 safeguarding enquiries, working with individuals and multidisciplinary teams to safeguard vulnerable adults.
- Contribute to team duty functions, providing essential cover and support as required.
- Independently manage a caseload of complex cases, delivering tailored interventions to meet individual needs.
- Collaborate with related services and community groups to enhance service delivery and client outcomes.
