What you will do:

- Assess the physical and mental well-being needs of service users, then plan, implement, evaluate, and monitor care in collaboration with service users, carers, and the Multidisciplinary Team (MDT).
- Implement care that addresses the mental, physical, spiritual, psychological, and social needs of service users, being mindful of age, culture, race, gender, ethnicity, social class, sexuality, and disability.
- Administer medication and have a strong grasp of contraindications and side effects, providing accurate information to service users in an understandable format.
- Ensure all risk assessments and care plans for service users are current and available.
- Encouraging service users towards self-determination and choice, while working within legal, ethical, and professional boundaries.
- Work within a framework of policy, professional, and legal standards at all times, including knowledge of the NMC Code of Professional Conduct and child protection protocols.
- Maintain high standards of written communication, accurately recording events and observations in the E-PJS according to Trust policies and the NMC Code of Professional Conduct.
- Foster collaborative relationships with colleagues to support open communication and effective MDT working.

Requirements:

- Must be registered with the HCPC.
- A valid driver’s licence is required to ensure you can meet the demands of the job and provide care efficiently.
