Social Worker - Hospital Discharge Team, Hastings, UK. Salary: £38 per hour, Contract Duration: 12 months

Our Mental Health Hospital Discharge Countywide team in Hastings is seeking a Social Worker to join us for a 12-month fixed term. As a Social Worker, you will have the opportunity to support individuals via a unique partnership within Sussex involving acute hospitals, the Sussex Partnership Foundation Trust, and voluntary organisations. Your role will focus on ensuring safe, creative discharge plans by placing individuals at the heart of decision-making.

What you will do:
- Conduct assessments to facilitate appropriate and safe discharge plans.
- Build and maintain strong relationships with clients, families, and multidisciplinary teams.
- Undertake home visits and coordinate with community resources for seamless transitions.
- Document and report on progress, ensuring compliance with social work standards.
- Work collaboratively with stakeholders to develop and implement discharge strategies tailored to individual client needs.
Experience we are looking for:
- You must be a Social Worker registered with Social Work England, with ASYE completion.
- Strong track record in building positive relationships with clients and professionals.
- Self-motivated and adept at managing your own workload.
- Ready to meet travel requirements associated with outreach and home visits.
- Dedicated commitment to ongoing training and personal development.
