What you will do:

• Complete Regulation 24 viability assessments as part of a duty rota

• Conduct comprehensive Full Connected Carers assessments

• Offer support and supervision to our Connected Carers

• Manage and complete the necessary review processes

• Spend 1–2 days a week in our Seaham office for team integration and duty purposes
