What you will do:
- Collaborate with multi-disciplinary teams including Social Care Assessors, Nurses, Therapists, and the MPFT Home First Team across various hospital and community settings.
- Conduct comprehensive Statutory Social Care Assessments and develop support plans tailored to individual needs.
- Perform Mental Capacity Assessments and facilitate Best Interest Meetings.
- Manage section 42 enquiries in line with the Care Act (2014) and Mental Capacity Act (2015).
- Handle Deputyship Referrals and actively participate in Continuing Healthcare Decision Support Tool Meetings.
- Work independently and as part of a team to effectively support service users.
- Utilize strong communication and IT skills to streamline operations and deliver efficient care.
