Job summary:
The post-holder will be required to work within the multidisciplinary team structure of the Community Mental Health Team to provide support to service users who experience severe and enduring mental health problems. You will have a small caseload and take responsibility for the day-to-day coordination of the multi-disciplinary team's clinical activities. Ensuring that clinical priorities are set, the highest standard of care is maintained, and the team caseload is regularly reviewed. You will be required to supervise and support members of the clinical staff and provide expertise on care coordination responsibilities and other aspects of day-to-day clinical operations, including statutory requirements and complex care issues. You are expected to make valuable contributions to multidisciplinary team meetings. Additionally, you will provide strong clinical leadership for the team and work closely with care coordinators to establish and maintain high clinical care standards, ensure that the quality of care provided is continually informed by the best evidence-based practice in conjunction with the Team Manager and Consultant. The post holder will assist in the screening of referrals and help coordinate and participate in referral, allocation, and discharge meetings. You will take responsibility for allocating caseloads in a manner that takes full account of the needs of the service user and expertise or skills available within the team. You will be familiar with the data or performance requirements of the Community Mental Health Team and be able to ensure that these are met by the team.
Requirements of the Community Psychiatric Nurse:
- Degree or equivalent in Mental Health Nursing.
- Experience working as a Mental Health Nurse.
- NMC Registration.
