What you will do:
- Assess the needs of service users and develop comprehensive care plans tailored to each individual's requirements.
- Collaborate with other healthcare professionals to ensure a multi-disciplinary approach to user care.
- Implement therapeutic interventions to enhance user independence and quality of life.
- Accurately document case management information for effective monitoring and planning.
- Act as a liaison between service users, carers, and different service providers for coordinated support.
- Provide guidance and support to families and carers, empowering them with strategies to aid user care.
The Occupational Therapist plays a lead role in coordinating the care and support for users and their carers or families pulling in the expertise of other professionals as required. You will also be responsible for the accurate and comprehensive inputting of case management information for monitoring and planning purposes. You will contribute to an efficient and cost-effective health and social care service to service users, carers and their families, using appropriate resources from Social Services, Health, the Council, community and non-statutory organisations.
