What you will do:
- Conduct complex Occupational Therapy assessments for service users with increased needs, particularly within the crucial first 12 weeks post-discharge.
- Lead on care package optimisation, aiming to reduce double-handed care where suitable.
- Manage and support Occupational Therapy Assistants, focusing on case delegation and supervision.
- Triage referrals from various hospital and community settings, ensuring urgent manual handling and equipment needs are promptly addressed.
- Collaborate with NHS and Adult Social Care teams to bridge service gaps and guarantee continuity of care.
- Offer expert input on equipment provision beyond the NHS Therapy team’s remit and support discharge planning from care homes and out-of-area cases.
- Contribute to service development by enhancing data capture workflows and facilitating training via the Care Academy.

Requirements of the Occupational Therapist:
- A degree or equivalent in Occupational Therapy.
- Recent experience within the UK as an Occupational Therapist.
- Previous experience working within the NHS.
- Registered with the HCPC.
