What You Will Do:

• Undertake Care Act assessments to support adults’ needs in the community.

• Carry out Mental Capacity Act assessments and make Best Interest decisions.

• Lead and support on safeguarding enquiries, advocating for vulnerable individuals.

• Attend and contribute to multidisciplinary and Best Interest meetings.

• Collaborate with external agencies to develop comprehensive, person-centred care plans.

• Accurately maintain records and reports, ensuring accountability and compliance.

• Offer guidance and support to service users and families, promoting independence and well-being.
