What you will do:

  • Provide a timely response to safeguarding concerns, perform section 126 enquiries, and collaborate with partners to develop safe care and support plans.
  • Assist individuals experiencing crises through comprehensive assessment, support, family liaison, and commissioning emergency placements or care packages when necessary.
  • Conduct outcome-focused assessments, ensuring full participation of service users and/or their carers in the process.
  • Manage urgent requests for equipment and address manual handling concerns.
  • Promote individual wellbeing through Information, Advice, and Assistance offered by our Wellbeing Matters Service.
  • Review or assess the needs of individuals already known to Adult Services where circumstances have changed, necessitating a new assessment.
  • Develop comprehensive Care & Support Plans in collaboration with citizens to enhance their wellbeing and social engagement within the community.
  • Undertake assessments in line with statutory duties under relevant legislation.
  • Plan and commission services to aid adults with care and support needs, monitoring and reviewing care plans created through a partnership with carers and citizens.
  • Maintain up-to-date electronic case files and relevant documentation, ensuring accurate performance management records and statistical data.
  • Engage in supervision and personal development reviews to identify training needs and maintain social work registration.
