What you will do:

• Conduct assessments for adults in need of support and develop appropriate care plans.

• Collaborate with a multidisciplinary team to ensure comprehensive care for service users.

• Keep accurate and up-to-date records of interactions and interventions.

• Provide crisis intervention when necessary and support vulnerable adults through challenging times.
