Key Responsibilities

• Deliver specialist Occupational Therapy interventions to patients with recent stroke or neurological diagnoses in community settings.

• Independently manage a complex caseload, providing person-centred care using advanced clinical reasoning and evidence-based practice.

• Offer professional advice and education to patients, families, carers, and health/social care colleagues.

• Ensure smooth care transitions, coordinating with other services to support continuity and patient safety.

• Work collaboratively within a multi-disciplinary team, contributing to the achievement of service targets and performance indicators.

Essential Requirements

• HCPC Registered Occupational Therapist.

• Proven experience working within neuro-rehabilitation or stroke care.

• Strong clinical skills and understanding of best practices in neurological conditions.

• Excellent communication, organisation, and collaborative skills.

• Driving licence not essential – all qualified professionals are welcome.
