What you will do:

Report to the Team Manager or Senior Practitioner, leveraging partnerships with customers and carers to provide tailored support.

Conduct comprehensive assessments, reassessments, and support planning under the National Health Service and Community Care Act 1990.

Offer valuable information, advice, and support to customers who do not meet the Council’s eligibility criteria.

Implement risk-based strategies that incorporate safeguarding measures at every turn.

Engage in dynamic multi-disciplinary teamwork to achieve holistic solutions.

Prepare needs-led assessments and support plans that are both effective and efficient.

Utilise IT systems to enhance the quality and reach of your work.
Experience required:

Proven experience in completing assessments and reviews.

Background in working with adults, older people, or individuals with physical or learning disabilities.

Adept at working in community or hospital settings.

Experienced in developing comprehensive needs-led assessments, reassessments, and support plans.

Competent in using IT systems to streamline and improve service delivery.
