What you will do:

- Act as the primary point of contact for new referrals to the social services department, providing immediate advice and support.
- Conduct assessments and determine the level of risk and need for each case, collaborating closely with other professionals.
- Develop, review, and update care plans to ensure they remain relevant and effective.
- Liaise with families, carers, and other agencies to ensure a joined-up approach to supporting individuals.
- Ensure timely and accurate case recording in accordance with local policies and procedures.
- Provide expert guidance to colleagues and newly qualified social workers within the team.
