What you will do:

- Undertake comprehensive assessments of adults with social care needs, considering their physical, emotional, and social wellbeing.
- Develop and implement personalised care and support plans aimed at preventing hospital admissions, reducing dependency, and enhancing quality of life.
- Work collaboratively with multi-disciplinary teams, including health professionals, housing, and voluntary sector partners, to coordinate effective interventions.
- Provide advice, guidance, and advocacy to service users and their families, ensuring their voices are central to decision-making processes.
- Maintain accurate and timely records in line with professional standards and organisational policies.
- Participate in safeguarding procedures to protect vulnerable adults from abuse or neglect.
- Contribute to continuous service improvement initiatives within the community prevention framework.

Person Specification:

- Registered Social Worker with the Health and Care Professions Council (HCPC).
- Proven experience in adult social care, preferably within community or prevention settings.
- Strong assessment, communication, and interpersonal skills.
- Ability to work independently and as part of a multi-disciplinary team.
- Knowledge of relevant legislation including the Care Act 2014 and safeguarding adults protocols.
- Commitment to promoting equality, diversity, and inclusion in practice.
