What you will do:
- Work collaboratively with medical and social services teams to ensure safe and efficient hospital discharges

- Liaise effectively with multidisciplinary teams to ensure seamless and person-centred care

About You:
- Previous experience working within an Integrated Hospital Discharge Team is essential
- Familiarity with the Eclipse system would be advantageous
- Demonstrated ability to operate efficiently in a dynamic and high-pressure environment
- A full UK driving licence and access to a vehicle is required
- Strong interpersonal and communication skills to engage with diverse populations
- A degree in Social Work or related discipline, with a commitment to continuous professional development
