What you will do:
- Conduct Care Act 2014 assessments for adults with learning disabilities, using a strengths-based and person-centred approach.
- Develop, implement, and review care and support plans in partnership with service users, families, and other professionals.
- Undertake safeguarding enquiries in line with Section 42 of the Care Act, ensuring service user safety and dignity.
- Carry out Mental Capacity Act (MCA) assessments and participate in best interest decision-making processes.
- Work collaboratively with multi-disciplinary teams including health, housing, education, and voluntary sector partners.
- Contribute to transition planning for young people moving from children’s to adult services.
- Participate in Continuing Health Care (CHC) assessments and reviews as appropriate.
- Maintain accurate, up-to-date, and legally compliant case records and documentation.
- Prepare and present professional reports for panels, legal proceedings, or care reviews.
- Promote independence, inclusion, and well-being in line with the principles of the Care Act and personalisation agenda.
