What you will do:

• Conduct assessments and develop treatment plans for patients on the Acute Stroke unit.
• Work collaboratively with multi-disciplinary teams to deliver high-quality care.
• Provide education and support to patients and their families regarding stroke recovery.
• Monitor and evaluate patient progress, adjusting treatments as necessary.
• Maintain accurate and timely documentation for all interactions and treatments.

The candidate must be HCPC-registered and hold a full UK driving licence, as the role requires travel between sites.
