Adult Social Worker – Services Transition Team (Adult Services Transition), Medway, Kent
Hourly Rate: £38 | Contract: Ongoing (Initial 3-month contract with potential for extension)
Working Pattern: Part-time (3 days per week) | Location: Hybrid (Medway-based)
Essential Requirement: Access to a car with business insurance cover

Key Responsibilities:

• Undertake comprehensive assessments in accordance with the Care Act 2014 and the Mental Capacity Act 2005.

• Develop and implement person-centred support plans for young adults aged 18–25 transitioning from Children’s Services.

• Work collaboratively within a multi-disciplinary team to ensure cohesive and holistic care delivery.

• Maintain accurate, timely, and professional records in line with statutory requirements and best practice standards.

• Provide guidance and support to families and carers, facilitating access to appropriate community services.

• Engage in team meetings, contributing to service development and continuous improvement.

Candidate Requirements:

• Qualified Social Worker with current Social Work England registration.

• Demonstrable experience in adult services and transitions work.

• Ability to manage caseloads effectively in a hybrid working environment.

• Must not have held a permanent role at listed councils within the last 6 months due to MoC restrictions.
