What you will do:

• Conduct effective screening and triaging of referrals in the Triage Hub.
• Perform comprehensive OT functional assessments tailored to client needs.
• Provide minor aids and adaptations that enhance daily living activities.
• Conduct Disabled Facilities Grant assessments for major adaptations to improve clients' living conditions.
• Offer support and guidance to Social Care Assistant staff, ensuring cohesive teamwork.
• Drive to various locations as needed, using your own car to easily reach clients and service sites.
