What you will do: Conduct thorough assessments of children with disabilities and their families to identify and address their needs.Develop and implement personalised support plans in collaboration with families and multi-agency partners.Carry out safeguarding interventions and regular reviews to ensure the wellbeing of the children.Support the transition of children and young people into adulthood, working closely with Young Adults Teams.Engage with partners, including health professionals, special schools, and families, to deliver holistic services.
