What you will do Care Act Practice: Undertake Care Act assessments, reviews, and support planning for adults with a range of needs.Mental Capacity Work: Complete Mental Capacity Assessments and make Best Interest decisions in line with legislation.Complex Caseloads: Manage complex cases involving safeguarding, hospital discharge, and community‑based interventions.Multi‑Disciplinary Collaboration: Work closely with nurses, occupational therapists, physiotherapists, and wider community services.Person‑Centred Practice: Promote strength‑based approaches that enhance independence and wellbeing.Care Coordination: Arrange and review care packages and connect individuals with community resources and support networks.Safeguarding: Participate in safeguarding enquiries and manage identified risks effectively.Recording & Reporting: Maintain accurate, timely case records and professional reports.Carer Support: Undertake carer assessments and provide guidance to appropriate support services.Hospital Discharge: Contribute to safe and effective hospital discharge planning where required. Qualifications / RequirementsEssential Qualified Social Worker with a recognised social work qualification.Registered with Social Work England.Experience working within Adult Social Care.Strong knowledge of the Care Act 2014, Mental Capacity Act, and adult safeguarding procedures.Ability to manage complex caseloads independently.Excellent assessment, communication, and partnership‑working skills.Full UK driving licence and access to a vehicle where required. Desirable Experience working within integrated NHS or community teams.Background in hospital discharge or community rehabilitation.Understanding of Continuing Healthcare (CHC) processes.Familiarity with NHS or local authority case management systems.
