This is an excellent opportunity for a practitioner who enjoys fast-paced decision-making, varied casework, and delivering timely outcomes for adults with a wide range of needs.This role sits at the front door of Adult Social Care, providing triage, assessment, and initial support planning for individuals referred into the service. You will be working with a diverse caseload, often involving complex and time-sensitive situations, requiring strong professional judgement and a person-centred approach.Key Responsibilities Undertake timely triage and assessment of adult social care referrals.Deliver strengths-based, person-centred assessments using remote and digital tools where appropriate.Apply the Care Act 2014, Mental Capacity Act, and Making Safeguarding Personal principles in practice.Identify levels of need, risk, and appropriate support pathways.Work collaboratively with multi-disciplinary teams and external agencies.Maintain accurate case records and ensure high-quality documentation.Contribute to safeguarding discussions and decision-making processes.Work independently while remaining an effective team contributor within a supportive service. Applicant Requirements Qualified Social Worker with current Social Work England registration.Experience within Adult Social Care, ideally in assessment, triage, or front-door services.Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding frameworks.Ability to manage a fast-paced caseload and prioritise effectively.Confident in undertaking remote assessments and using digital communication tools.Strong communication, analytical, and decision-making skills.Enhanced DBS preferred or willingness to obtain.
