Key Responsibilities

Carry out specialist physiotherapy assessments and deliver timely interventions for patients on the Frailty unit
Apply acute care skills effectively in front-door admissions, rehabilitation, and similar clinical areas
Work collaboratively as part of a multidisciplinary team to provide comprehensive care planning
Develop targeted treatment programmes aimed at improving mobility and functional independence
Maintain accurate clinical documentation and contribute to case reviews and healthcare meetings

Requirements

Current HCPC registration as a Physiotherapist
Full driving licence to support travel for home visits and between sites
