What you will do:

  • Provide a professional social work service to adults, their families, and carers in accordance with the Care Act 2014 within Hammersmith and Fulham.
  • Engage with individuals, families, and communities to assess needs, employing a variety of interventions to promote choice, control, and independence.
  • Demonstrate expert and effective practice in complex situations while managing higher levels of risk.
  • Assist in the development of support plans using strength-based approaches and available resources to meet identified outcomes.
  • Offer clear guidance on Personal Budgets and Direct Payments, facilitating informed decision-making.
  • Actively promote wellbeing and independence, helping individuals and carers regain skills and confidence.
  • Facilitate contingency planning with families to pre-emptively address complexity and changing circumstances.
  • Ensure compliance with the Care Act 2014, supporting the use of advocacy services when necessary and conducting mental capacity assessments.
