What you will do:
- Work with older adults to assess their needs and develop personalised care plans.
- Collaborate with multidisciplinary teams to deliver comprehensive mental health support.
- Provide crisis intervention and support for older adults experiencing mental health challenges.
- Liaise with families and carers to ensure holistic support and care coordination.
- Maintain accurate case records and ensure compliance with relevant legislation.
- Use your expertise to contribute to service improvement initiatives.

Requirements:
- Must be registered with Social Work England.
- A minimum of five years post-qualification experience is required.
- Evidence that the Assessed and Supported Year in Employment (ASYE) was completed within two years of qualifying.
- At least two years of experience as a Social Worker outside agency settings, ideally within an older adults' mental health team.
- A valid driving licence is essential.
- Able to start within four weeks.
