Social Worker - Mental Health Team within Newham: earn £32.01 hourly in this rewarding and ongoing full-time role. What you will do:

• Conduct comprehensive community care assessments including Care Act assessments
• Undertake capacity assessments and contribute to tribunal processes
• Engage in safeguarding assessments to ensure the protection of vulnerable individuals
• Facilitate hospital discharges to help clients transition smoothly back into their communities
• Collaborate closely with a team of mental health professionals to provide quality care and support
