Working within a multidisciplinary team, you’ll play an essential role in providing personalised, intensive support to help people make meaningful and lasting changes.

What You Will Do:

• Deliver high-quality case management for individuals with three or more complex needs, ensuring effective, person-centred outcomes.

• Collaborate within a multidisciplinary team, developing and implementing tailored support plans.

• Conduct home visits and community outreach, promoting engagement and sustainable progress.

• Maintain accurate records and assess progress, adapting interventions where necessary.

• Liaise with healthcare professionals, local services, and partner agencies to ensure a holistic and coordinated approach.
