Purpose of Job:
Your role is to provide assessment, care management, and social work services to users and carers, supporting mental health needs and recovery. Implement local policies, coordinate with voluntary and statutory resources, and collaborate with various teams to deliver outstanding service.

What you will do:

  • Collaborate with the multidisciplinary team to assess needs and establish eligibility for services under Fairer Access to Care and the Care Programme Approach.
  • Provide social work services for adults with mental health issues and their carers, ensuring comprehensive support.
  • Manage and review care plans in collaboration with service users, carers, and other workers to meet assessed needs.
  • Act as a care coordinator as part of the care programme approach, ensuring effective delivery of services.
  • Coordinate care packages in compliance with the Council’s Financial Regulations.
  • Utilise information management systems for documentation and retrieval to maintain high information accuracy.
  • Participate in duty rota systems within the multidisciplinary team environment.
  • Undertake safeguarding adult enquiries, maintaining the highest standards of care and protection.
  • Receive professional supervision and assessment against agreed standards and targets to enhance personal and professional development.

Requirements:

  • Registered with Social Work England
  • Degree/Equivalent in Social Work
  • UK Qualified experience working as a Social Worker
