What you will do:

- Conduct assessments and reviews as outlined under the Care Act 2014 to ensure the best outcomes for adults with learning disabilities.
- Serve as a safeguarding lead, coordinating and undertaking critical safeguarding enquiries to protect vulnerable individuals.
- Manage and support adults within the community, tailoring care plans to enhance their quality of life.
- Participate in the duty rota, primarily based in the office, ensuring a seamless flow of service delivery.
- Maintain accurate case records and ensure compliance with statutory requirements.
