Social Worker – Discharge to Assess Team in Solihull, UK, earning £28.40 Hourly. As a part of Solihull Metropolitan Borough Council within the Adult Care & Support Division, the Discharge to Assess Team is seeking a social worker to provide vital support to the Out of Hospital and Supporting Independence Department. What you will do:

  • Complete thorough long-term assessments and manage discharges for individuals on Discharge to Assess pathways.
  • Engage in safeguarding duties and manage complex cases within tight timeframes.
  • Exercise decision-making in positive risk-taking and collaborate with multiple agencies.
  • Apply detailed knowledge of relevant legislation such as the Care Act 2014, Mental Capacity Act 2005, and the Mental Health Act 1983.
  • Communicate complex and sensitive information effectively, both verbally and through detailed report writing.
  • Respond proactively to challenging situations using strong interpersonal skills.
