What you will do:

- Manage patient discharge processes efficiently within a tight timeline of 3 to 6 days.
- Carry out comprehensive assessments and attend crucial meetings by travelling to Wexham Park Hospital as required.
- Undertake safeguarding responsibilities for all patients potentially affected by neglect or abuse.
- Act in a duty capacity once a week, contributing to the team's overall functioning.
- Leverage any existing experience with LAS for smoother transition and role execution.
